[ech] APIs for ECH

Folks,

I'd really appreciate comment on these APIs as my next steps
for ECH are to add more test code and to update the client and web 
server integrations done earlier.

As your fine comments are likely to lead to API changes, if
those come later, that'd likely mean having to re-do a bunch
of test code and integration so how's about being nice to me
and reading and commenting on this now(ish)? :-)
